Continuous care

We will strive to assign one of our highly qualified and trained carer’s to each case where possible and aim to work seamlessly with Case Managers and their Clients to provide and implement quality packages of care.

Our Care Manager will conduct regular visits and calls with the Client so they can discuss their care and we can share ideas.

At Home Helpers Care we are committed to establishing a robust long-term care planning strategy that will maximise independence and fulfil needs and wishes leading to an improved quality of life for our Clients.
